The assessment portion of the 5 Behaviors program is fairly simple. Participants will receive an email with a link to an introductory video explaining the assessment and a link to the survey itself. The survey takes about 30 minutes to complete. There are two parts of the assessment: the individual and the team assessments. Individual responses are not reported by name in the team profile; they are grouped with responses of other team members.

Individual DiSC assessment

Impact of your DiSC styleThe DiSC assessment is like other Everything DiSC assessments. It asks respondents to respond to statements regarding their normal behavior. They respond on a five-point ordered response scale, indicating their level of agreement with a statement such as “I love meeting new people.”

The responses are used to score the eight DiSC scales: D, Di, i, iS, S, SC, C, CD. The assessment uses adaptive testing methods, so respondents might get a few additional questions to clarify a score.

Results from the DiSC assessment are presented to each individual in their report. The DiSC model is also explained and used throughout the team report.

Team assessment

Two subsections are included in the team assessment. The first covers how team members engage in specific healthy behaviors. For example, participants respond to how often team members solicit one another’s opinions during meetings. The twenty items from this subsection are used to create the scores on the Five Behaviors scales.

The second subsection is focused on evaluating the team’s culture. The questions are more value-based and let members respond to what behaviors they find appropriate for their team. It’s an opportunity to express the desire for more or less expression of specific behaviors.

After the assessments have been completed by each team member (minimum of three team members are required to create the final report), the administrator can run the reports and distribute printed copies or email out PDFs to team members.
